We will call you for a review of your Hypertension annually and this will usually be around the month of your birthday. We will require a blood pressure reading and in some cases a blood test, especially if you are on medication.
We have a Patient POD within the waiting room at Hawkinge and a BP machine in the waiting room at Elham should you be asked to provide a blood pressure reading at anytime. Patients can also self monitor and can provide the readings to the practice via eConsult or the practice can provide a text message where the home readings can be recorded and returned to the practice and these will be filed directly into your notes and then reviewed by a clinician.
