Medication Reviews

Medication reviews are carried out by GPs and by our clinical pharmacist. This is a key role for pharmacists as part of the multidisciplinary approach to care. During the reviews, pharmacists can address medicines optimisation, safety and medicine adherence issues thereby improving the clinical effectiveness of medicines taken by patients. Many problems with usage or due to chronic condition can be avoided by monitoring the effects of therapy, making recommendations to change therapy and modifying medication where necessary.
