Total Triage – What It Means for You
A New Way to Request Appointments
🗓️ Starting 1st May, our practice will be moving to a Total Triage system
What is Total Triage?
Most appointment requests (except nurse appointments) will now be assessed by a clinician before being booked. This means we can make sure the right care is provided, at the right time, by the right person. Where appropriate our trained Care Navigators may be able to book you directly with one of our local services within our community i.e PCN Minor Illness, Pharmacy.
How Do I Request an Appointment?
1. Online (Preferred): Complete a short form via - SEE LINK BELOW
2. By Phone: Call the practice. Our team will complete the form with you over the phone.
Why Are We Doing This?
- Patients will be prioritised based on clinical need, not how early they call.
- It helps us manage increasing demand safely and fairly.
- We can reduce wait times and improve outcomes.
Who Can Help Me?
Our care navigation team is trained to guide you through the process and ensure you receive the right care, whether that’s a GP, nurse, or another health professional.
